Bloomfield Department of Health will offer the new monovalent COVID-19 vaccines. Moderna vaccines for ages 12 and older will be available through the health department.
Eligibility requirements to receive the vaccination include:
• Residents of Bloomfield, Caldwell, and Glen Ridge
• Ages 12 and older residents
• Underinsured/ uninsured
• Medicaid recipients or NJ Family Care Part A recipients
• American Indian/ Alaskan Native

Please contact your primary care provider and/or pharmacies if you have private insurance.
If uninsured/ underinsured or under age 12, vaccines are available through the Essex County Mobile Health Clinic: https://essexcountynj.org/mobile-health-clinic. Another option for this group is to visit the nearest Federally Qualified Health Center (FQHC); to locate the nearest FQHC, visit https://healthapps.state.nj.us/fhs/cphc/cphcSearch.aspx

In-Home COVID-19 Vaccines for New Jersey Residents!
Bloomfield Department of Health will offer vaccines to homebound residents in Bloomfield, Caldwell, and Glen Ridge. For additional information, please call 973-680-4058.
Who is Eligible for the COVID-19 Vaccines?
Bloomfield Department of Health is no longer offering COVID-19 vaccines to everyone. However, all NJ residents six months and older are eligible for a COVID-19 vaccine.
